Please submit a request for a Facility License below. The License will be sent by email from Essentrics via BoldSign.* Please make sure the facility representative reads it carefully and follows the instructions for signature submission. A signature will only be considered valid when done electronically via BoldSign. Note that upon signing the License, the facility representative will receive an email with the subject line “Completed.” This email does not serve as License approval. 

Once the License has been approved, a confirmation email will be sent to the facility representative and the facility name will be added to the list of Approved Facilities for Live Virtual Classes on Essentrics.com.
